Eligibility Criteria:

  • Persons living with HIV/AIDS (PLWHA)
  • Proof (documentation) of HIV+ and referral from medical provider
  • Medicaid eligible
  • Must be 18 years of age or older

Services Provided:

  • A bilingual program available to members that are Spanish speaking
    • HIV general medical services
    • Nursing services
    • Sick call visits
    • Case management services
    • Art therapy
    • Support groups
    • Food and nutrition services
    • Social services (housing, legal, family support, etc.)
    • Counseling for HIV risk reduction
    • Directly observed therapy program (DOT) for greater adherence assistance
    • Harm reduction services
    • Referrals for dental services and sub-specialty care including GYN care
    • Mental health and psychiatric services
    • Pastoral counseling for any client requesting such services
