HUD Supportive Housing Program for Families is located at 306 Lenox Avenue, 2nd floor. This program has the capacity to serve English and Spanish-speaking clients. 

Our services include:

    •Substance use assessment and counseling

    •Mental health assessment and counseling

    •Medical monitoring and medication adherence

    •Apartment unit maintenance/environmental safety support

    •Legal assistance to help regarding immigration issues

    •Housing referrals and placements

    •Entitlement assistance and advocacy

    •Linking/referrals for legal assistance

    •Monthly case management home visits

    •Family reunification

    •Escorts to medical, entitlements, housing and all other appointments

Brooklyn HUD Scattered-Site Housing Program

Funded by HUD, our “Independent” Scattered Site Housing Program provides Brooklyn-based permanent housing for single adult HASA clients.  This program serves individuals almost completely ready for independent living.  The program is the lease-holder of each apartment for the first 12 months, with the expectation that each client will graduate the program 12 months after admission, at which time the lease is moved into the client’s name and supportive services cease. 

Therefore, this program is a good setting for those clients who need short-term supportive services to stabilize their functioning—adherence to medical protocols, budgeting and paying bills, maintaining good nutrition, etc.—and who can then move into fully independent living without the support of home visits or ongoing case management. 

During participation in the program, each client receives a weekly home visit from a masters-level case manager.  By contractual obligation, the apartments are located in Bedford/Stuyvesant and surrounding Brooklyn neighborhoods.  Clients are charged 30% of their SSI/SSD/VA benefits to partially pay the monthly rent (HASA pays the rest) with no rent portion charged if the client’s sole income is Public Assistance. 

Our services include:

    •Weekly home visits

    •Case management geared to each client’s needs

    •Referrals to health care, and assistance in maintaining treatment protocols

    •Harm Reduction education and counseling

    •Assistance in budgeting and paying bills fully and in a timely manner

    •Reimbursement of up to $20 per month of the client’s phone bill (until graduation)

    •Reimbursement of up to $40 per month of the client’s utilities bill (until graduation)

    •A monthly rent-portion bill mailed to the client’s home

    •Notices of rent arrears mailed to the client’s home, as needed

    •Care coordination with other providers, including health care, methadone maintenance, psychiatric and specialty providers
