Our Supportive Housing Division provides over 588 scattered-site and congregate units for PLWHA, including women and children, undocumented immigrants, and the mentally ill. Supportive Housing clients receive a crucial array of supportive services including primary care support, treatment education, recreation, mental health services, structured socialization, advocacy, and substance use counseling.

Health Homes (Formerly Known As COBRA)

The goal of the Health Home program is to reduce cost while increasing quality and efficiency in New York’s Medicaid Program. The Health Home program will improve the health care provided to members of the Medicaid Program and will save money by reducing preventable hospitalizations, emergency room visits and unnecessary care. Health Homes provide a care coordination model for high-cost, high-need Medicaid recipients. It is a care management service model whereby all of the professionals involved in a member’s care communicate with one another so that all needs (medical, behavioral, health and social service) are addressed in a comprehensive manner.

  • Advocacy for housing and entitlements
  • Comprehensive Care Management
  • Coordination of your care and health promotion
  • Help transitioning from inpatient to other settings
  • Support to you and your family

FROST'D HUD HOUSING

The FROST’D HUD Scattered Site Housing Program provides transitional housing for HIV/AIDS positive individuals with a history of or whom are currently homeless. For a 12-24 month period the program works to ready clients for independent living and to successfully manage their illness. The apartments are located in Brooklyn neighborhoods.  This program has the capacity to serve English and Spanish-speaking clients. 

Services Provided:

  • Monthly home visits and case management services by program staff
  • Case management with goal of increasing clients’ self-sufficiency and ensuring housing stability
  • Facilitate connection and adherence to medical care through referral and education counseling
  • Advocacy to establish and maintain entitlements
  • Harm Reduction education and counseling
  • Reimbursement of up to $60 per month for client telephone and utilities 
  • Ongoing care coordination with internal and external providers

NY / NY III HOUSING

The NYNY III program provides 70 units of permanent scatter site housing and supportive services to chronically homeless single adults who are living with HIV/AIDS and co-occurring serious and persistent mental illness, substance abuse disorders, or both.  Chronically homeless is defined as an unaccompanied disabled individual who has been continuously homeless (living on the streets or in a shelter) for at least one year. Our program works with clients to stay healthy and remain housed, using a multi-disciplinary team approach. 

  • Services Provided:

    • Monthly (at minimum) intensive case management services in the home
    • Medical monitoring , medication adherence, and care coordination
    • Mental health counseling and referral
    • Substance use counseling and referral
    • Activities of Daily Living (ADL) skills training
    • Referral to ADHC, detox, or other supportive services as appropriate
    • Referral to Voc Ed for vocational training or education as appropriate
    • Apartment care and maintenance assistance
    • Advocacy and case management
    • Linking/referrals for legal assistance and other entitlements
    • Escorts to medical, mental health, entitlements, and all other appointments

Foundation house north and foundation house south

Foundation House North and Foundation House South provide emergency transitional, congregate housing for up to 180 days of care.  Foundation House North is located in Washington Heights and has 10 rooms (4 rooms for single adults and 6 rooms for adult couples) the maximum capacity is 16 adults.   Foundation House South, located in the Lower Eastside, has 14 rooms for single adults.

Services Provided:

  • Safe, secure shelter for a maximum of 180 days
  • Three nutritious meals per day, 365 days per year
  • Onsite intensive case management
  • Assistance in securing entitlements and advocacy
  • Individual and group counseling and workshops geared toward 1) strengthening adult daily living (ADL) skills of cooking, cleaning, budgeting and shopping; 2) enhancing self-efficacy intended to achieve productive relationships with HASA workers, workers in other agencies, and personal contacts; and 3) developing productive conflict resolution skills
  • Substance use assessment and harm reduction counseling
  • Referrals to health care and aggressive advocacy to ensure clients’ entry into and maintenance in health care services
  • Care coordination with other providers, including health care, methadone maintenance, psychiatric and specialty providers
  • Onsite medical assessment within a week of admission and ongoing as needed
  • Psychosocial assessment geared towards assessing appropriate housing placements
  • Referrals to supportive housing programs and (for some clients) assistance in obtaining independent apartments

Brooklyn HUD scattered site housing

Brooklyn HUD Scattered Site Housing provides 32 units of transitional housing for homeless single adults living with HIV/AIDS who are HASA eligible. For a 12-24 month period, the program works to prepare residents for independent living and successfully managing their illness. The goal of the program is to graduate residents after 12 months, thereby transferring the lease into the resident's name and reducing the need for supportive housing case management services. The program is ideal for homeless adults living with HIV who are in need of short-term supportive services to stabilize their health and living conditions. Apartments are located in Bedford/Stuyvesant and surrounding Brooklyn neighborhoods. Residents must pay 30% of their income toward rent; no rent portion charged if the resident's sole income is Public Assistance.

  • Monthly case management home visits
  • Medical monitoring and medication adherence
  • Referrals to health care and assistance in maintaining treatment protocols
  • Harm Reduction education and counseling
  • Life skills training including vocational, employment and educational services
  • Assistance in budgeting and paying bills fully and in a timely manner

The Transitional Housing Program provides a total of 78 units of scatter site apartments to single adults. The ultimate goal of this program is to prepare clients for more independent and permanent next step housing settings, within two years.
